During your first appointment you will be one on one with a Doctor of Physical Therapy. Components of your initial evaluation with Dr. Rakestraw will generally include:
A thorough medical history review
Discussion of your current issue or issues
Discussion about your goals for therapy
A comprehensive examination and discussion of examination findings
Discussion of how physical therapy can help you specifically and what your physical therapy treatment plan will entail
Discussion of any questions you have

Physical therapy evaluations and follow up treatments are 55 minutes.

Primary Motion is an out-of-network provider. That means you pay for your physical therapy sessions up front and if you wish to submit to insurance for reimbursement we will provide you with a detailed invoice or “super bill” that includes diagnostic codes and treatments codes of the services rendered for you to submit.
At Primary Motion we feel strongly that the care you receive and the goals you have are not controlled by an outside entity, and that you receive personalized care by a specialist who can spend their full time and focus on you. We want to help you achieve your potential without any barriers!
Please contact your insurance company to understand your out-of-network physical therapy benefits, as well as the reimbursement process, since many will vary. Each client is responsible for working with their own insurance company to navigate the reimbursement process and determine reimbursement rates.

Physical therapy evaluations cost $210 and follow-up treatments are $170. You will be charged at the time of service.

No. As of July 2023, you are able to access physical therapy care directly without a physician's prescription.

Most clients will need anywhere from 4-12 sessions.

Cancelling within 24 hours of your appointment time results in a 50% charge. A “no show” (missing your appointment entirely or not alerting your provider of a cancellation) will result in a full charge for your session. (For example if your appointment is at 7am on Monday, you will need to notify us by 7am on Sunday of a cancellation)

You may use the 2 parking spots closest to the door in front of the building. You may use any of the spots along the north side of our building. If those spots are occupied, we recommend any of the parallel spots on Church Street between Park and the W&OD trail.
